The Department of Maxillofacial Surgery was created in 1932 with the organization of the Institute for Advanced Training of Physicians. Its leader was Professor Alexander Eduardovich Rauer, an outstanding Russian scientist who had extensive experience in treating the wounded in the First World War. At that time, wounded people with facial defects were often left on the battlefield, because... there was no experience in feeding and treating them.
Alexander Eduardovich became interested in the treatment of this category of victims and created the first department of maxillofacial surgery at the Institute of Traumatology and Orthopedics in Moscow, on the basis of which the department was later organized. His colleague was Nikolai Mikhailovich Mikhelson, together with whom they summarized their vast practical experience in the monographs: “Fractures of the jaws and damage to soft tissues of the face,” M. 1932, “Plastic surgery on the face,” M. 1943, republished in 1954. At that time, maxillofacial surgery was classified as general surgery. After the death of Prof. A.E. Rauer in 1948, the department was headed by Prof. Mikhelson Nikolai Mikhailovich, who defended his doctoral dissertation in 1938 on the topic: “The use of cadaveric cartilage in the clinic.” For the monograph “Plastic surgery on the face” he and prof. A.E. Rauer was awarded the Stalin Prize. Professor Mikhelson N.M. published the monograph “Reconstructive operations of the face”, M. 1965.
The associate professor was Mikhail Pavlovich Sheftel, known for his work on the use of the Filatov stem to replace extensive tissue defects. In 1963, after the death of Prof. Mikhelson N.M. The department was headed by his student, Dmitrieva Valentina Stanislavovna, who defended her doctoral dissertation in 1959 “Radiation damage to the maxillofacial region.”
In 1963, CITO (Central Institute of Traumatology and Orthopedics) moved to a new building on the street. Priorov, and a new institute is being organized in the old building - the Central Scientific Research Institute of Dentistry - TsNIIS. Some of the department's teachers resign and move to CITO, the department is renamed the department of “Surgical Dentistry”. War participant, prof. Dmitrieva V.S. dramatically renews the teaching staff during 1964-1967. are retiring Ph.D. Silaeva Anna Stepanovna, who was involved in the treatment of congenital facial defects, Ph.D. Frigov Rudolf Markovich, who supervised the treatment of the injury, Ph.D. Petrov V.L., who worked on issues of surgical dentistry. New teachers from among graduate students, having quickly completed their master's theses, are expanding the range of treatment of surgical diseases of the face and oral cavity. Assistant Savitsky Vladimir Aleksandrovich defends his dissertation on neurofibromatosis of the face and neck, Bogatsky Vladimir Aleksandrovich - “Surgical treatment of true progeny”, Nerobeev Alexander Ivanovich - “Traumatic osteomyelitis of the lower jaw.”
Professor Dmitriev V.S. She was distinguished by her energy and public activity; up to 15 applicants from all regions of Russia were working on their dissertations at the same time. The department began to actively develop a new form of training - on-site cycles, in which almost all teachers took part and conducted on-site operations and gave lectures for a month. Every year, 2-3 on-site cycles were organized on various topics - treatment of jaw injuries, inflammatory processes, congenital deformities of the face and jaws. Together with the Department of Oncology of the Central Institute for Advanced Training (CIU) of doctors, under the leadership of prof. Yu.V. Fallileev conducted cycles on head and neck tumors. Employees of the department took an active part in all dental congresses and conferences in the country, and prof. Dmitrieva V.S. was invariably elected to their presidiums. Under her leadership, about 100 dissertations, mostly candidate's, were defended, which made it possible to dramatically increase the level of specialized care in the country. The leadership of the department in the country in matters of postgraduate education was unconditional.
In 1971 prof. Dmitrieva V.S. moved to work at the Department of Dentistry at the Patrice Lumumba University, and the Department of CIU was headed by Prof. Ermolaev Igor Ivanovich, transferred from MMSI (Moscow Medical Dental Institute). Preserving the traditions and breadth of coverage of all sections of the specialty, and active participation in visiting cycles, prof. Ermolaev I.I. being the country's chief dentist, he paid a lot of attention to the organization of this specialty and, in particular, oncology of the maxillofacial area. During this period, an oncologist Ph.D. came to the department. Matyakin Evgeniy Grigorievich as an assistant.
Due to the appearance of a large number of patients with extensive facial tissue defects after tumor removal, the department began to pay considerable attention to replacing these defects. Nerobeev A.I. and Matyakin E.G. studied the TsNIIS archive for the previous 20 years and calculated that to fully eliminate the defect, at least 6-10 operations are required over a period of 2 to 3 years. The main tissue restoration occurred through the use of the Filatov stem, through its repeated migrations. The TsNIIS clinic (the main base of the department) was headed by prof. Khitrov F.M., the founder of reconstructive surgery using the stem in our country, laureate of the Lenin Prize for his work on plastic surgery of the pharynx and esophagus, who did not accept new surgical methods appearing abroad. These methods are based on the formation of plastic material taking into account the blood supply to the integumentary tissue, which ensures their direct movement to the defect. In 1975, in the city clinical hospital No. 36, a department of emergency surgical dentistry with 60 beds was opened and the head invited the assistant of the department Artsybushev V.I., who, retaining 0.5 of the assistant’s rate, offered to move to the new base A.I. Nerobeev. For 10 years until 1985 in this hospital, along with the development of methods for treating the consequences of injury and inflammation - necrotic phlegmon, osteomyelitis, sepsis, etc. new methods of reconstructive operations were mastered, taking into account the axial blood supply to the tissues being moved.
In 1978 prof. Ermolaev I.I. died suddenly and was invited to become the head of the department in 1980. Colonel Vladimir Andreevich Sukachev, who was previously the chief dentist of the missile forces. The department was located at 2 bases - in hospital No. 36 and in TsNIIS, where the priority topic was orthognathic operations on the jaws, on which Sukachev V.A. published the monograph “Atlas of reconstructive operations on the jaws”, 1984.
Since 1978, associate professor Nerobeev A.I. established close contact with the newly created surgery center, under the leadership of Academician B.V. Petrovsky. Department of Microvascular Surgery, headed by Prof. Krylov V.S. Nerobeev A.I. performed numerous experiments on free tissue transplantation using microvascular anastomoses and in 1979 was included in the operating team that performed the first in Russia free transplantation of a skin-fat flap from the groin region to a defect in the temporal region in one stage. In 1983, Nerobeev A.I. defended his doctoral dissertation in general surgery on the topic “Plasty of extensive tissue defects of the head and neck with complex flaps with an axial vascular pattern.”
Prof. Sukachev V.A. in 1986 he went to work at TsNIIS and Doctor of Medical Sciences. Nerobeev A.I. in 1987 he headed the department, which subsequently changed its name several times, due to the lack of nomenclatures in the country - plastic, maxillofacial surgery. The department received its final name – “Plastic and Maxillofacial Surgery” in 2010, after the approval of these specialties by the Russian Ministry of Health.
In 1985, the management of TsNIIS invited prof. Nerobeeva A.I. to head the department of reconstructive surgery of the face and neck, and in 2005 to head the entire TsNIIS clinic; he remained at 0.5 of the head’s salary. To this day, the department is located at the bases of Hospital 36 (now the Inozemtsev City Clinical Hospital) and the Central Research Institute of Infectious Diseases (now the National Medical Research Center "TsNIISiChLKH"). In 2022, a private clinic was added to them - “ART Clinic”, which deals with aesthetic operations.
During the time of heading Professor Nerobeev A.I. 44 candidate and 9 doctoral dissertations were defended at the department. Most of the department's employees are former graduate students: Doctor of Medical Sciences. Malakhovskaya Vera Ivanovna, Doctor of Medical Sciences Shurgaya Tsitsino Mikhailovna, Ph.D. Surovykh Svetlana Viktorovna, Ph.D. Bolshakov Mikhail Nikolaevich. Associate Professor Evgeniy Isaevich Garelik and prof. Yuri Sergeevich Egorov joined the department staff after the disbandment of the Department of Microsurgery of the Academy of Postgraduate Education.
The main directions of scientific and practical activity of the department are the restoration of lost tissue in a short time using modern microsurgical equipment.
This includes operations on the peripheral nerves of the head and extremities, for vascular malformations of the head and neck, neurofibromatosis, combined and combined defects. Employees of the department took part in the organization of all 8 national congresses on plastic surgery, participated in all congresses of the European Association of Maxillofacial Surgeons since 1992, and the Russian Society of Maxillofacial Surgeons.
