Threads for the face - everything you need to know: says plastic surgeon Georgy Chemyanov
Threads are one of the most popular face lift techniques today. Many proclaim it a panacea for all age-related ills - they tighten the oval of the face, get rid of jowls, nasolabial folds, and a double chin. A real alternative to plastic surgery! But there are also dissatisfied patients who believe that threads are wasted money: the result is not so bright, and serious rehabilitation is required.
asked Candidate of Medical Sciences Georgy Stanislavovich Chemyanov, a member of the Russian Society of Plastic, Reconstructive and Aesthetic Surgeons (ROPRS) and the International Society of Plastic Surgeons (IPRAS), to help figure it out
Dr. Chemyanov has been working as a plastic surgeon for 16 years, and with threads for more than 10 years. And he doesn’t just stage them, but is an expert trainer, conducting master classes all over the world - from Moscow to Miami and Seoul.

What are the pros and cons of the thread technique?
Of course, the main advantage is that this is a cosmetic procedure: not a single incision, not a single seam. Through punctures, the doctor passes threads under the skin, which are fixed in the soft tissues, tightening and strengthening them.
The main disadvantage is the duration of action. If, after tightening the oval, we did not cut off excess skin, as we do during plastic surgery, the result will not be as long-lasting. But for cosmetology, a year and a half is how long the effect of thread techniques lasts - a decent period and a good result.

Has anything changed in the 10 years you've been working with threads?
Aesthetic medicine is constantly evolving, including thread techniques. Now everything is moving towards reducing invasiveness and trauma. To make procedures easier for both the patient and the doctor. And the rehabilitation period is shorter.
The majority of horror stories and myths about threads are, of course, already outdated. Previously, installing threads required, one might say, a small operation - thick, non-absorbable threads were inserted under the skin, which left behind some irregularities. Now the doctor uses cannulas, and the procedure is usually without bruising. Both the insertion technique and the thread itself are being improved.
Almost a month ago we presented the third generation Aptos thread. Hyaluronic acid is added to the composition of absorbable (also called biodegradable) threads. This is a revolution in aesthetic medicine.
Now, in the process of dissolving the threads, the quality of the leather becomes better. Due to hyaluronic acid, it is moisturized, the formation of collagen around the threads is stimulated, not to mention the fact that the rehabilitation period is reduced. There are already a number of proven positive effects of using such threads.

Who are threads shown to? Maybe it’s better to go straight to plastic surgery?
It is important to understand that threads are a cosmetic procedure, and it is indicated for “younger” patients when excess skin has not yet appeared. The biggest mistake is to install threads on patients whose facial oval has already become very swollen and excess skin has appeared in the neck area. Even a non-specialist understands that without cutting anything off, just tightening the skin, its excess will not go anywhere.
But when there are only the first signs of disruption of the contours of the face - nasolabial folds, the cervical-mental angle is no longer so sharp, then we can create a frame under the skin with threads, slightly tighten and fix the tissue.
In addition to excellent results, this will also prevent further age-related changes. We fix the tissues for a year and a half, and they can no longer fall as they would without the “frame”.

Some patients complain that the threads leave “dimples” – points from knots and tension.
In my opinion, most of these negative aspects - unevenness on the face, threads that did not work - are primarily a consequence of incorrectly selected techniques for the patient.
The threads are different and differ from each other both in the time of resorption and in the material in the composition. Of course, the quality, cost, and biodegradation period vary. There are European threads (this is polylactic acid) and Asian threads (polydioxanone), which dissolve faster and the result from them lasts less.
But a lot depends on the hands of the master. Any procedure can be performed poorly – not only thread surgery, but also hardware, injection, and plastic surgery.

By the way, about the devices. Some also offer a lifting effect. What is better - threads or devices?
I would recommend choosing a technique not on your own, but trusting a specialist. During the consultation, the doctor will evaluate the soft tissues of the face, age-related changes and recommend the most suitable technique for you. The threads and devices are not competitors to each other; they have different effects.
Let's think logically. Using hardware techniques, we stimulate collagen deep in the skin to improve its structure. When carrying out the injection procedure, we try to hold the soft tissues with a liquid preparation. Against this background, the thread is the most practical movement.
A dense thread with notches or some other anchor elements, with notches that hold the fabric, really creates a frame. Its effectiveness is clear. It is not hypothetical; we are establishing a thread that holds the soft tissues together.

Is it possible to perform plastic surgery after threads?
This is one of the most frequently asked questions. And there are enough myths on this topic, even among experts. I operate a lot, and, accordingly, since I use threads, I also operate on patients “with threads.”
In short, all cosmetology makes life difficult for us surgeons. And devices, and fillers, and threads - everything leaves a mark in the tissues. They become denser and more difficult to work with.
But this is the reality of today: rarely does a patient see a surgeon without previous cosmetic interventions. Yes, such an operation may be a little more difficult for the surgeon, but that is his concern. Therefore, I declare responsibly: techniques with absorbable threads are not a contraindication to plastic surgery.
