Pastosity is moderate swelling of the skin and subcutaneous tissue. The term comes from the Latin word pasta - “dough”. It is characterized by pallor of the skin, decreased elasticity and resembles the structure of dough upon palpation. It is a precursor to edema.

The symptom often causes aesthetic concerns. May be a transient problem and develop into fixed tissue swelling.

Local links in the development of pastiness

Pathogenesis has the same links as with edema. The difference lies in the degree of changes formed.

So what happens:

  1. An increase in hydrostatic pressure in the capillaries, which leads to increased filtration of water from the cell into the intercellular space.
  2. Reduced hydrostatic pressure in tissues. Increased release of fluid from the blood into the tissues.
  3. Violation of osmotic balance with an increase in salts in the interstitial fluid, which attract water molecules from the cells.
  4. An increase in water in the intercellular space due to a lack of proteins in the blood plasma, which retain water molecules in the bloodstream.

  • nephrotic syndrome (protein is lost in the urine).

Clinical classification of pastiness

It is based on two principles: causal and organ.

Classification:

  1. Heart. The contractile activity of the myocardium decreases and venous stagnation occurs. Gradually, the veins expand compensatoryly, and some of the blood enters the intercellular space.
  2. Renal. A dual situation occurs: loss of protein and decrease in oncotic pressure. The fluid enters the surrounding tissues, the volume of circulating fluid decreases and the renin-angiotensin-aldosterone system turns on. All this leads to sodium and water retention, aggravating the edema syndrome.
  3. Inflammatory. One of the components of the inflammatory reaction is tissue swelling due to local vasodilation and sweating of the liquid part of the blood with the formation of exudate.
  4. Allergic. In response to contact with an allergen, mediators are released, which dilate blood vessels and increase their permeability. There is a release of fluid into the surrounding tissues.
  5. Cachectic. Associated with protein deficiency in food. The albumin fraction retains water molecules in the bloodstream, and when they are deficient, the liquid enters the intercellular space. There is an expression “to swell from hunger”, which is appropriate in this case.
  6. Endocrine. When thyroid hormones decrease, all types of metabolism slow down. Excess glycosaminoglycans accumulate in the skin, sodium and fluid retention occur. Specific swelling of the face (myxedema) occurs.

How to relieve pain

Traditional medicine treats excess fluid accumulation depending on the cause of its development. Treatment regimens often include • restriction of fluid intake; • reduction of salt and sodium in general; • taking diuretics. All of them have a lot of side effects. Diuretics, for example, can reduce the amount of potassium, which can negatively impact your heart function and make the problem worse.

Diagnosis of pastosity

There is a McClure-Aldrich test for increased tissue hydrophilicity. 0.2 ml of isotonic sodium chloride solution is injected intradermally. Normally, the formed blister resolves within an hour. The more pronounced the hydrophilicity of the tissues, the faster the swelling resolves.
