Every day, when getting ready for work, we spend some time in front of the mirror, putting ourselves in order. This is required by our mental perception of aesthetics. Having “liked” ourselves in the mirror, we calmly go to work and completely devote ourselves to it, we feel confident, we are comfortable, we are sociable, we are successful, we return home in a good mood to our family, to our loved ones and devote ourselves to them, we are self-sufficient and we have something to give to our loved ones... but... There comes a moment when we have to spend more and more time in front of the mirror... unproductive... dissatisfied... and we are already late for work, getting annoyed along the way with literally everything, with the “boring” bosses , “worthless” employees, “boring” clients. The world is turning upside down. We get tired, lose success at work, return home “out of sorts,” and lash out at people close to us. Life imperceptibly pushes us to the margins. Sadly? And the most offensive thing is that we repeat to ourselves: “What can you do, age...” Of course, nothing can be done with age.

However, there is always a way out, and those who really want to achieve something are looking for this way out, and those who don’t want to are looking for excuses for their own inaction. Let’s ask ourselves the question: what does it take to overcome yourself and go “beyond the flags”?

Of course, you need to be realistic and understand that miracles do not happen in the world. Trying to be critical of the bold statements of cosmetologists and plastic surgeons, but completely neglecting the capabilities of modern cosmetology and plastic-aesthetic surgery in our age is irrational.

Plastic surgery is a huge, multifaceted and unfinished section of SURGERY. It would take a very long time to list its capabilities, goals and objectives. The site is dedicated to a private section of plastic surgery - AESTHETIC surgery, i.e. aimed at maintaining a person’s external aesthetics, eliminating congenital and acquired defects, and eliminating age-related and other involutional changes in appearance. Let me emphasize that aesthetic surgery is not intended to change your appearance! This is not her task.

Surgical techniques aimed at eliminating these problems have been known for a long time, there are a lot of them and they are constantly being improved. There are no universal methods; there is a rational approach to the problem: the possibility of combining methods and multi-stage treatment of a particular problem.

Is changing the shape of your face, breasts or nose a thing for rich people? Plastic surgeons say the opposite. In our city, Evgeny Sventitsky, who has been working in this industry since the 60s, is considered the guru of plastic surgery. Now his followers and students work in Tver. The Karavan+ weekly managed to talk to one of them, Mikhail Poletkin.

– Mikhail Vladimirovich, how long have you been working in plastic surgery?

– Since 2002. I am a general surgeon by training; from 1998 to 2002 I practiced general surgery.

– Why did you decide to become a plastic surgeon?

– In fact, it’s a matter of chance. In the early 2000s, Tver did not need ordinary surgeons. And there were almost no plastic ones. A vacancy appeared, I went there and stayed. I won’t say that this has been my calling since childhood. But little by little it took root. At first, of course, it was wildness. From conventional surgery to plastic surgery – these are completely different questions. It just didn’t fit in my head.

– What is the fundamental difference between people who need a regular surgeon and those who need a plastic one?

– The requirements are different. There are patients suffering from some kind of disease, but here people come with questions of beauty, not health. And then, as a novice surgeon, this was completely incomprehensible to me; I perceived such patients as abnormal. This is where you, a young woman, should enlarge your breasts, you will take better care of your health. Now I already understand that a person’s health is not only in his physical well-being, but also in his psycho-emotional one. And for this, appearance is of great importance.

– So, in order to be calm, some people need to adjust their appearance?

– Yes, if a person’s external data does not correspond to his self-perception, then discomfort appears. Even life as a whole can turn out unsuccessfully just because a person has an ugly nose or ears. People beat themselves up, it happens. And your health may suffer as a result. This is very important for children. For example, a child has lop-eared ears. At school his peers bully him. Complex for life. And you just need to have a small operation to correct it in childhood.

– It turns out that plastic surgery is a desire for beauty. But there are cases when such an operation is necessary. For example, when a person has an accident, his face is disfigured.

- Yes, this happens. But here it is not so much plastic surgery that works as reconstructive surgery. As a rule, injuries that occur during an accident are not just associated with scars or damage to the face, there is already a violation of the anatomical shape. More serious surgery works, and plastic surgeons don't usually do that.

But damage in the form of scars, injuries on the face or body is our job. We, of course, cannot eliminate the defect completely, but we can reduce it to a minimum.

“There is a demand for noses”

– What do Tver residents most often turn to a plastic surgeon for? By the way, are these most often men or women?

– It’s difficult to even compare percentages here. A couple of men may apply in a year, the rest of the patients are women. Of course, they begin to apply at a certain age, after 40 years. When women feel that they are already starting to grow old. Self-confidence is lost. They notice things in themselves that others may not even see. This is laxity of the skin, veins, face.

– What about more serious interventions – changing the shape of the nose and ears?

– This is already the lot of the younger contingent. The ears are mainly children and students. Noses are young girls when they need to change something in themselves. Sometimes the hump of the nose is beautiful, but... it’s not like everyone else’s. And the girl begins to perceive herself somehow differently. After that, he comes to me asking if this hump can be removed. Technically this can be done. But does a girl imagine herself with a different nose, and is she doing it right? There are noses that don't suit people at all. Such a miniature female face and a huge nose inherited from dad. There really is a dissonance here. And often people want to remove some feature. Everyone thinks that our Slavic noses are straight, like small potatoes. And any deviation - a small hump, a pointed nose - means that it is not like everyone else. They come to the surgeon. They undergo nose surgery... and lose their exclusivity. They become like everyone else. And whether they like it later or not, we don’t know.

– In such cases, do you dissuade the patient from surgery?

- Yes. If this is not an aesthetic defect, but just the structure of the nose, then the conversation will primarily be about “she’s good, you’re pushing yourself, it’s just a complex.” I try not to carry out such operations.

“I remember a few years ago there was a popular program on a music TV channel where they talked about people who had surgery to look like show business stars. Do people come to you with similar requests?

– No such demands were made specifically. But an example of, say, a nose like a star’s was shown in the photograph. But the nose there was not average; it was technically difficult to make it look like the picture.

- Why is it difficult?

- Man is still not plasticine, and we are not gods. Therefore, we can satisfy some desire, but we will not promise that the nose will look like the photograph. They don't often come with such requests. Our city is small.

– How many patients do you have per year?

– On average, this is 200–300 operations. Now there is a demand for noses. This is due to the fact that in the 90s there was a demographic failure. And then - growth. These girls, who were small then, have grown up, and now they want to change something in their appearance.

– Are these complex operations?

– Painstaking, done blindly. Not in an open way, but closed, under the skin. And most importantly, the postoperative period flows uncontrollably. So we performed the operation, put the bones together, and put on a plaster cast. And we don’t know what’s there. In ten days we are just opening everything. And let's see what happened. There were cases when it was necessary to redo it. And not even to redo it. This is called corrective surgery, I warn you about this in advance that it may be necessary. I had such a case when the patient refused to make a correction. She was probably offended.

– Are plastic surgeries expensive?

– You see, in Russia plastic surgery arose as some kind of polyp. Regular doctors don't like her. Because of its privacy, payment. And it’s not just in our country, it’s all over the world. But my teacher Evgeniy Vasilievich Sventitsky made plastic surgery more accessible. People from all over Russia come to him for operations.

– I also have the opinion that plastic surgery is for the rich.

– Yes, that’s what the majority thinks. Therefore, sometimes a person has a desire to change something in himself, but does not turn to us for fear that operations are expensive. But that's not true. It all depends on the specific case.

– Ten years ago you came to plastic surgery and are not going to leave?

– Let’s put it this way: so far the clients and I have found each other, and then we’ll see.

Blepharoplasty

When is eyelid blepharoplasty needed?

In some cases, there are indications for this operation at a young age (consequences of injuries, birth defects, etc.), but most often adult patients resort to it. As we age, collagen fibers deplete and the skin loses its elasticity. Due to the lack of subcutaneous tissue near the eyelids, the tissue begins to sag, and due to the preservation of fat cells in the preorbital area, small hernias form. Minor changes can be eliminated with the help of some cosmetic procedures, but only competent surgical intervention will help get rid of serious age-related changes.

The presence of inflammatory processes, increased eye pressure and glaucoma are strict contraindications to eyelid surgery, but in any case you should consult an ophthalmologist before the operation.

Which type of blepharoplasty is right for you?

Depending on the location of the problem areas, the doctor may recommend upper or lower blepharoplasty (depending on which eyelid will be manipulated). If your case requires large-scale intervention, the specialist will suggest performing circumferential eyelid surgery (complex correction of the upper and lower eyelids).

  • Classic eye blepharoplasty is performed through incisions in the crease of the upper eyelid or along the border of the ciliary edge of the lower eyelid. After complete healing, traces of the operation are almost invisible. This approach is recommended in cases where pronounced negative skin changes extend beyond the eye sockets and affect adjacent areas.
  • If the elasticity of the patient's skin is preserved and age-related changes are not pronounced, the operation is performed transconjunctivally (an incision is made along the inner surface of the lower eyelid). In this case, the trace of the cut is absolutely not visible.
  • Laser blepharoplasty is recommended for minor skin changes. In difficult cases, this method will not produce the desired effect.

Modern techniques make it possible to achieve the desired result almost painlessly. For example, it has become possible to turn the “Asian” eye shape into a “European” one.

Description and consequences of the operation

To carry out the intervention, the doctor administers local anesthesia. During the plastic surgery process, you will be conscious, but will not feel any discomfort or pain. During the manipulations, the surgeon will excise excess skin, distribute fatty tissue, fix the eyelid muscles, and apply a cooling bandage.

In the postoperative period, unfortunately, the appearance of edema and small hematomas is inevitable. This is a normal reaction of the body. However, by the end of the first week they gradually disappear, and you will be able to appreciate the doctor’s skill.

The effect of the operation lasts, as a rule, up to 10 years. However, this period may vary in one direction or the other depending on the patient’s lifestyle and individual characteristics.
