Introduction

Individuals of the Central Asian type are characterized by their own characteristics of the development of involutionary changes. A flatter facial skeleton and features of the fat packets in the upper and middle thirds lead to the development of deep nasolabial folds, drooping corners of the mouth, and the appearance of pronounced marionette lines.

Clinical case

The patient, 57 years old, complained of sagging skin, drooping soft tissues of the middle third of the face, the appearance of deep nasolabial folds and folds on the skin of the upper lip at a distance of 1 cm from the commissure, drooping corners of the mouth, and the severity of marionette lines. She had not previously received aesthetic treatment.

Materials and methods

It was decided to carry out a comprehensive correction that could improve the visualization of those areas of the face that the patient complained about. There were no contraindications for the procedure.

After collecting anamnesis and examination, a procedure plan was drawn up:

bolus fixation of soft tissues in the cheek-zygomatic area (prevention of further development of gravitational ptosis),
carrying out bioreinforcement of soft tissues using the “honeycomb” technique (restructuring of the dermis and epidermal-dermal junction, improvement of skin tone, indirect lifting effect),
leveling of nasolabial folds, folds on the skin of the upper lip, correction of the contour of the corners of the mouth and marionette wrinkles.

Stylage filler from Vivacy was used for the procedure. The products in this line are unique due to the patented HA stabilization system (IPN-like technology), the presence of mannitol in the composition and a variety of options for solving all aesthetic shortcomings that cosmetologists encounter.

Procedure protocol

Stage 1. Skin treatment with 0.05% chlorhexidine solution and application anesthesia with Acriol Pro cream for 45 minutes.

Stage 2. After repeated treatment with chlorhexidine, we make markings in the cheek-zygomatic area: the first line starts from the lateral edge of the eyebrow, descends vertically to the intersection with the lower jaw, the second - from the wing of the nose to the cochlea, the third - a virtual line emphasizing the shape of the cheekbone.

We place the injection points as follows: the first - at the intersection of the first two lines, the second - 1.5 cm medial to the first, the third - 1 cm medial to the first (optional point, we look at the structure of the cheek-zygomatic region), the fourth - 1 cm 5 cm medial from the first.

We choose a product with high viscosity and elasticity - Stylage XL. The volume of filler injected into the point is 0.1–0.2 ml, the total amount is 0.5 ml on one side.

Injection level: supraperiosteal, medial and lateral SOOF. Before administration, we perform an aspiration test by pulling the syringe plunger toward ourselves on the count of “ten.” There should be no blood in the needle or in the cannula of the syringe. After injection, carefully distribute the filler.

Stage 3. Bioreinforcement using the honeycomb technique. We begin to apply markings from the temporal region to the edge of the lower jaw, without going into the skin of the upper lip. The technique consists of hexagons adjacent to each other, open in the infraorbital region. The rib length is 13 mm needle length, the needle diameter is 27G. The injection level is the reticular layer of the dermis, the border is the dermis-hypodermis. We carry out the procedure with Stylage M, 1 ml on each side, on average 0.01 ml per injection.

Stage 4. Correcting nasolabial folds. The drug Stylage L, linear retrograde injection, fan-shaped - at the wing of the nose and one injection below, level - the lower layer of the dermis, border - dermis-hypodermis. On the right - 0.35 ml, on the left - 0.3 ml. At the level of the corner of the mouth, we make an injection along an arc, through the commissure, and deposit 0.01 ml at the level of the corner with the aim of turning it upward.

Stage 5. We pierce the contour of the lip along the border of the skin and dry mucous membrane, the technique is linear retrograde, there is a white stripe above the surface of the needle, which indicates a superficial insertion. Needle 27G, 13 mm, amount of drug - 0.01 ml per side.

Stage 6. Working with puppet wrinkles. The technique is linear retrograde, according to the “fan”, “lattice” scheme. The drug Stylage L, the average amount is 0.2–0.25 ml on each side.

Stage 7. After completion of the procedure, carefully distribute the drug according to the natural contour of the face. It is possible to apply Traumeel S ointment, and it can also be applied at home.

After the procedure, the patient should limit facial expressions, physical activity and visiting the pool on the first day, as well as visiting the sauna and solarium for 3-4 weeks.

The use of Stylage fillers allows you to obtain a natural and prolonged result with minimal severity of aseptic inflammation and the development of adverse events.

Correction results

Using the technique described above, we were able to eliminate all the aesthetic shortcomings that the patient complained about. In this case, we used 4 ml of Stylage (1XL, 1L, 2M). The patient was very pleased with the result.
