Often people come to me with a pronounced disproportion, their noses seem out of place. In such cases, you have to take a particularly careful approach to solving the problem of shape, since the “new” nose must fit perfectly into the “architecture” of the appearance and not violate the geometry of the face as a whole. As for the “technical” issues that I mentioned just above, they made it possible to solve a number of surgical problems that many of my colleagues have faced and continue to face. In my work, I strictly follow those aesthetic canons and my author’s “tricks”. All this makes the results of my operations, I will say without false modesty, impeccable.

Everyone's idea of ​​what a nose should look like is different. Do you encounter any difficulties in communicating with a patient who has decided to undergo rhinoplasty? Do you immediately find understanding?

Svetlana Pshonkina:

We immediately come to a common denominator if the patient’s fantasies on the topic “what my nose should look like after surgery” do not diverge from the possibilities of nose surgery. If during the conversation I understand that the patient’s ideas are far from reality, then I begin to explain why it is impossible to do as he wants. If my words seem unconvincing, which is extremely rare, then I prove them right with the help of computer modeling. And if at this stage mutual understanding is not reached between me and my patient, then we simply say goodbye to him. I am not his surgeon, and he is not my patient. And this is the normal practice of plastic surgeons.

That is, you can refuse such patients?

Svetlana Pshonkina:

Yes, I refuse patients. I have no shortage of patients, so I can afford to carry out corrections for those who really need them, and for those who have adequate expectations from the operation. But the trouble is that there will also be specialists who will follow the patient’s lead and make a “nose to order.” The consequences of this are known - dissatisfaction with the results and, as a result, despondency, depression and other negative emotions. And who is to blame for this? The patient himself or the surgeon who succumbed to the persuasion of this patient?

Svetlana Pshonkina:

I love my job completely and completely, so you can say that revision rhinoplasty is part of my favorite thing. Honestly, when I see complex noses, the desire to take it on and fix it increases significantly. Much more than doing primary rhinoplasty. Well, there is more satisfaction from such operations - you realize how significant your help is for the patient. After doing the work for other surgeons, my patients cry with happiness, realizing that the nightmare is over. And I am sincerely happy for each of them. I see my task not just to fix the nose, I want the lives of my patients to change for the better along with their nose.

Are you saying that rhinoplasty can change something in a person’s life?

Svetlana Pshonkina:

I'm afraid that I will take up all your time if I start telling the stories of my patients. Believe me, Hollywood is resting! When a person gets rid of complexes, he grows wings. And it’s not surprising that a confident person’s whole life changes!

Svetlana Pshonkina:

Alas, recently patients with complications after rhinoplasty are becoming more and more overweight. The patient you are talking about had a nasal injury since birth. Svetlana Pshonkina:

Of course you can! Summer means sun, fruits, positivity, our body works at 100%, so healing is easier and faster. Therefore, there is no reason to postpone the operation until the fall. Many of my patients had surgery by the summer, and many were booked for the summer. So the summer holiday season is not about me. A lot of work!

Is rhinoplasty combined with other plastic surgeries?

Svetlana Pshonkina:

Yes, rhinoplasty can be combined with breast surgery in various variations, abdominoplasty (mini-abdominoplasty, classic or endoscopic); it is often possible to combine nose surgery with minor operations on the face. When deciding whether it is advisable to perform a combined operation or not, I approach it individually.

Svetlana Pshonkina:

First of all, on his experience. Believe me, rhinoplasty is too complex to master its intricacies in 3-4 years. — Yes, it is believed that it is easier for men to be surgeons: they need to act decisively and confidently, and make decisions quickly. But women can also exhibit these qualities. This is rather a question of self-discipline and determination. Moreover, the majority of patients at aesthetic clinics are representatives of the fair sex, and who, if not a woman, can understand the desires of another woman?

What do you think is the most important thing in the work of a plastic surgeon?

“This is precisely to understand the very desires and expectations of patients and bring them into line with the capabilities of modern aesthetic medicine. A plastic surgeon must also be, to some extent, a psychologist. There is no way without this. Before undertaking a correction, you need to understand what motivates the patient, why he wants to change, and whether surgery will solve the problem.

Usually they come to me with an already formed picture of the upcoming changes. Sometimes these ideas differ from reality, and my task is to explain to the person why it won’t work out the way he wants. It also happens that verbal arguments do not seem convincing, then I use computer modeling. Well, in those rare cases when it is not possible to dissuade the patient, we have to say goodbye - if we did not reach agreement at the preliminary stage, then there is no point in working further.

So, “the client’s desire is the law” is not a rule for you?

Over my many years of practice, I have performed many operations, and all of them were successful. People left happy, but I also received satisfaction from my work. I don’t want to go against existing principles; I make corrections when they are really necessary and people adequately evaluate the results. Nowadays there are many surgeons who will fulfill any client’s wishes, but in the end the person does not get a new beautiful appearance, but complete dissatisfaction with the results - I have more than once had to correct the mistakes of other surgeons.

Not everyone likes to redo the work of another specialist...

— If you sincerely love your job, then doing it “from scratch” or correcting other people’s mistakes is equally interesting. The main thing is that the client is satisfied and gets the best result. And you know, patients with complex noses, who have already been in the hands of unqualified surgeons, react much more emotionally to their new appearance. I am sincerely happy for them and proud that I changed their lives for the better.

Since we're talking about difficult cases, can you share some interesting stories from your practice?

Just to continue the topic of revision rhinoplasty: one of my patients, Tatyana, since childhood had discomfort about her appearance due to a congenital injury to the nose. In 2000, she decided to see a surgeon, but, unfortunately, the operation was unsuccessful and her nose could not be fixed. When Tatyana came to me, I really wanted to help her, and from a medical point of view the case turned out to be interesting. After studying the medical history, photographs and communication with the patient, I made a reconstruction plan and performed the operation. Tatyana was very worried, but everything went extremely well. The woman recovered quickly, and when she first saw her new nose, she literally burst into tears of joy. It was very nice to know that I made a person happy.

Svetlana Yuryevna, probably, over your 20 years of practice, you have developed your own methods and techniques?

Of course, I have developed my own experience both in the field of rhinoplasty and in other areas of aesthetic surgery. Experience and practice allow us to choose the most effective ways to solve various problems. As for the “technical issues”, I would prefer to leave them secret, this is my personal merit. And from an aesthetic point of view, there are no secrets - I try to give harmony to my facial features and do not strive for fashionable ideals. Sometimes patients come with clearly disproportionate noses, and in this case I try to fit the new nose into the overall geometry of the face so that it does not look like a foreign body. I won’t be modest - all my “tricks” work and this proves the success of the operations I performed.

Probably the most concerning questions for a patient after rhinoplasty are how soon after the operation can the result be assessed and how long will it last?

- You're right. In most cases, this is exactly what we talk about during consultation. The first results of rhinoplasty can be assessed after 2 weeks, when the splint (or plaster) is removed. But at this stage it should be taken into account that while swelling still remains, the nose may lack miniature and elegance. However, in general, this is already the result and we can already talk about whether we like it or not. The final version of the nose is formed within 6 or even 12 months. Swelling completely subsides after 4-6 months. And only then can you breathe a sigh of relief, look at your nose and say: “I like it, I’m satisfied.”

Are there any nuances in preparing for surgery?

There are no special subtleties. The entire list of “not allowed” or “not recommended” that we use before any surgical intervention applies here. The one piece of advice I strongly encourage all my patients to follow is to relax and trust that everything will go well. A person's emotional state is very important.

Can a nose job be done at any time of the year or are there better seasons?

The seasonality factor is a product of our domestic media and the Internet. Aesthetic surgeries, and rhinoplasty in particular, can be performed at any time of the year. Such decisions should not be made by patients based on the outside temperature, mood and other minor factors. Let's take Latin America: it is always hot there, but this does not stop patients from realizing their dream and getting plastic surgery. Or Thailand, where medical tourism is now actively developing, people fly from all over the world to undergo surgery, although the average annual temperature here is +30 degrees.

Have you noticed changes in rhinoplasty trends in recent years?

It is clear that more patients are seeking repeat operations. There are, as I have already said, cases of poor quality work by specialists. But in general, the level of plastic surgery has increased in recent years. This means that people come again because they previously did not receive or did not receive what they planned. This may also be due to an insufficient level of preliminary preparation. The patient and the surgeon did not discuss, did not tell, misunderstood each other, and as a result - dissatisfaction with the results.

We know that you have undergone internships abroad several times. Is the foreign approach to rhinoplasty different from the Russian one?

Of course, the Western school is different from ours, but this does not mean that it is better there than here. I regularly travel abroad to see what progress our colleagues from the USA and Europe have achieved. It happens that I adopt some methods and technologies, but immediately reject others. Rhinoplasty is a very delicate operation, so the use of any know-how here must be carefully considered. And its main principle boils down to creating a harmonious profile with minimal trauma. Both the West and we are striving for this. But, in my opinion, we do it better.

And if you were choosing a surgeon for plastic surgery, what would you pay attention to?

— Rhinoplasty is not a light operation, and any surgical intervention is a serious step. Therefore, it is very important to find an experienced specialist. You won’t master all the nuances of such a complex operation in a year or two. I have been working in this field for decades and continue to discover something new. Of course, skill does not always come with experience; there are plenty of amateurs in rhinoplasty. Here we need to take a comprehensive approach, assessing many factors in choosing a surgeon.
