Face to face with Etermis: the mystery of the updated filler from Merz

Today we present a review of the updated line of Etermis fillers, which doctors may remember under the old name Glytone.

The updated line consists of two monophasic cohesive fillers based on hyaluronic acid (HA) in combination with mannitol.

Characteristics of Etermis fillers

Etermis – fillers with therapeutic effect

The duration and durability of the effect after filler injections is always an important topic for patients of cosmetologists. The topic becomes especially relevant for the so-called “hyaluronic acid eaters” - this is what doctors call patients in whom the effect of contour correction lasts for a short time, and hyaluronic acid is quickly eliminated from the body. Etermis fillers avoid this problem thanks to the presence of three components, cross-linked and non-cross-linked native hyaluronic acid and mannitol.

Thus, in patients who eat hyaluronic acid and in patients with “skin under stress” (those who are losing weight sharply, smoking, playing sports, actively sunbathing, vegetarians, etc.), the components in the Etermis line have a longer action and effect.

Etermis fillers are homogeneous (homogeneous), differ in the degree of reticulation (stabilization of HA using cross-links), due to which they have a smoother and more elastic consistency, and are easy to introduce and distribute (Fig. 2). The concentration of hyaluronic acid in the filler is 23 and 24 mg/ml (Etermis 3 and Etermis 4, respectively).

Cross-linking of hyaluronic acid reduces the ability of the filler to degrade and guarantees a long-lasting effect of the filler (Attenello, Maas 2015). Etermis allows you to obtain volume with clear contours in the lips, cheekbones, and along the contour of the lower jaw, which is important not only for women, but also for men (Fig. 3, 4). Monophasicity allows you to preserve the “stiffening ribs”. Typically, this was characteristic of biphasic fillers, but now modern developments and data on the safety of drugs have made it possible to apply this property to monophasic fillers.

The updated fillers also contain a special component – ​​mannitol. It provides a good safety profile, low hygroscopicity, strong tissue inertness, good density and viscosity, moisturizing and antioxidant properties.

Data from André and Villain (2017) indicate that the presence of mannitol in the filler reduces inflammation, tissue swelling, and degradation of HA by free radicals (Ohrem et al. 2014), and Korean scientists in 2016 found that the presence of mannitol increases the effect of the procedure for 6 months without affecting tolerability (Kim et al. 2016).

Patients observe pronounced changes after injections into the lip area - they note that in addition to additional volume, the lips acquire a brighter, more saturated color, become moisturized and well-groomed (Fig. 1).

Etermis fillers: evidence base

Open and observational studies with a blinded assessment of the effectiveness of both fillers showed good tolerability of the drugs and a stable long-term effect of the correction.

Etermis 3

Etermis 3 for the correction of nasolabial folds according to the Lemperle scale demonstrated results within 12 months. During this time, 47 patients (mean age: 55.7 ± 3.7 years) showed a decrease in the depth of the nasolabial folds and changes in the structure of the dermis, assessed using ultrasonography within 9 months after injection.

Patients were injected with 2.4 ml ± 1.1 ml of the drug into both nasolabial folds, 10 patients (21%) underwent additional correction, the average dose volume was 1.1 ± 0.4 ml in both nasolabial folds (Fig. 3).

Etermis 3 was well tolerated and patients reported high satisfaction when surveyed (Turlier et al. 2010).

Etermis 4

Similar results were obtained in an observational study of Etermis 4.

Over the course of two weeks, 35 women and 1 man were assessed for the condition and dynamics of correction of nasolabial folds.

After administration of Etermis 4, the average improvement on the Lemperle scale was:

1.61 for nasolabial folds (1.37 ml of filler injected)

1.50 for marionette wrinkles (0.93 ml of filler injected)

1.21 for wrinkles on the cheeks (0.73 ml of filler injected)

Moreover, all patients showed good tolerability of the drug (Clinical Study Report 2012).
