Surgeon Nikolai Kameko: “A plastic surgeon is an artist”

Nikolai Konstantinovich Kameko is the first plastic surgeon in Krasnoyarsk. He mastered the specialty of “surgical beauty” in the early nineties, but even now he remains the first. The best evidence of the quality of his work is his patients. But the profession of a plastic surgeon is risky precisely because of the patient’s perception of the final result of his work. How Nikolai Konstantinovich creates a new face, what is the priority in his work and how plastic surgery is developing - about this in our interview. - Plastic surgery, in my opinion, stands apart in medicine. On the one hand, there seems to be no vital indication for performing operations; you, plastic surgeons, do not treat diseases, but this is a surgical intervention in the human body. How not to make a mistake in choosing a doctor who will “make you beautiful”?

– The first thing a person who has decided to undergo plastic surgery should remember is to visit several clinics and talk to doctors.
Find out what his patients think about a particular specialist, read reviews on the Internet. Before undergoing surgery, you need to choose a doctor and take this very seriously. The most important thing is the doctor’s experience. Don’t be fooled by advertising - when the doctor says that he is the smartest, the best and criticizes other doctors. A good doctor will never slander his colleagues, and a good doctor does not need advertising. Patients are the best advertising, in this case word of mouth works. There are no doctors who do not make mistakes; those who do not operate at all do not make mistakes. Now in Krasnoyarsk there are about ten clinics that offer plastic surgery. We were the first to start, the Institute of Medical Problems of the Peoples of the North. In 1992, I was the first to be sent to the then Sverdlovsk to Professor Vissarionov, he now heads the Institute of Cosmetology in Moscow. And it’s funny to me now to hear from some surgeons who do plastic surgery that they have 15-20 years of experience. They may have been in surgery for a long time, but as far as plastic surgery is concerned, they have little experience, but we all know about each other. Patients just don't know. – Do you often have to correct the mistakes of your colleagues?
- A lot of.
I ask patients - who had it, how did they do it... - What are the main mistakes
-
they cut it off, they sewed it in the wrong place?
– The main mistakes are eversion of the eyelids, scars, gross asymmetry.
Many errors do not appear immediately. Patients, as a rule, do not return to the doctor who performed the operation; they look for another one who would already correct the mistakes. But we don’t always undertake alterations; I don’t know what was removed there or how it was done. I suggest the person wait, I convince him that time will pass, you will calm down, swelling and inflammation will go away, it will be much better, and then you will come back and see what can be done. Sometimes a person calms down and goes for a change to the doctor who did it; some, of course, stay with us, they don’t want to see that doctor. – What desires do people mostly come to plastic surgeons with?
– Nowadays, mostly young people come with breasts.
Here are the shapeless implants that we installed 15 years ago ( Nikolai Konstantinovich shows a bag of gel
). But the modern implants that are being installed now make a huge difference.
Previously there was a liquid gel, but the new generation of implants are filled with cohesive gel and this gives them special strength. If the operation is well done, then it is almost impossible to determine that there is something there, that the breast is enlarged. The breasts are soft, elastic, and you can never tell from the touch that there is an implant. Today, the endoscopic method of administration is used, and it is more gentle. A small incision is made in the armpit and no traces of the operation remain. Technologies are developing and everything can be done beautifully. – What about the safety of these implants?
– I was recently in Holland at an international conference of plastic surgeons, specifically on implants.
To crush this implant, you need 600 kilograms of pressure. Gel and silicone are of higher quality, so the implant is elastic and resistant to stress. – When women come to you for new breasts, do you fulfill their wishes or make adjustments?
– Those who have already formed a strong desire to enlarge their breasts or change their shape come.
There are women who say - I don’t need it and I will never get an implant to enlarge my breasts. Those who have already decided for themselves that they will undergo an operation come and they have been carrying this idea for more than one day, not one year. The only thing you have to convince is the choice of size. Some come and say “I want big breasts”; those who have large ones want small ones. I convince you that you need to make breasts that would be beautiful, and not look ugly. You can put a large implant on a thin woman - but it will be ugly, not aesthetically pleasing; it is much better to choose an anatomical implant of a natural shape. When choosing a size, the condition of the skin matters - it is easier for a woman who has given birth to place a large implant, but for a nulliparous woman, it can be very difficult to stretch the skin, and healing will be sluggish. There may be rejections, and you have to clean everything up. – How many women have been blessed with perfect breasts?
– To put it simply, over 18 years there have been about 500 women.
As for the face, what type is in fashion now, probably Angelina Jolie?
– When they come with a photograph, I immediately explain that it is impossible to make the same face as in the picture.
It is impossible to do the same for a second person. And the one who promises this is simply not being honest with the patient. And the result will be the same. – What about in films, when they make a double with exactly the same face?
– These are overhead silicone masks.
It is impossible to surgically create exactly the same face - there will still be differences, jaw, cheekbones, skull structure - it is impossible to make it identical to the chosen model. Everyone's skull is different. You can make it look similar, but not exactly the same face; no two people are the same. There are no symmetrical people; there will still be differences. If they insist, I say find another doctor. After all, if a person is dissatisfied with himself, he will never be satisfied, even after surgery. And then he’ll make further claims about why he doesn’t look like the photograph. That’s why I don’t take such patients, because they then start suing because they haven’t become like their ideal. If a person has a severe deformity, then no matter how you perform the operation, he will still be satisfied. Before the operation, the nose and ears are such that after the operation the person changes and is very happy with the result - this is another question. Each person has his own zest in his face, his own characteristics that must be taken into account. – How do you model a person’s future appearance on a computer?
- It's all nonsense.
You can do anything on a computer, but when you go to operate, cartilage, bones and skin behave differently, not like in a computer program. Moreover, we perform rhinoplasty in a closed way - through the nasal passages, so there are only hands and experience here. Another intuition. – There are two extremes in plastic surgery

it is at the height of fashion and in some circles it is considered good form to visit a plastic surgeon after a certain age, and on the other hand there are many cases when a person’s face has been disfigured...
– There are no ideal doctors, some mistakes still happen.
But you have to understand that the doctor is not doing this out of malice, not because he wants to specifically disfigure someone or do something bad. Maybe it doesn’t heal well in a person, maybe there are some characteristics of the body. There are many reasons. But we shouldn’t blame doctors for deliberate sabotage. – What procedure would you, as an experienced doctor, advise against doing?
– I would advise not to choose an inexperienced doctor.
If the doctor is experienced, he will advise which operation or procedure can be performed on the patient and which cannot. – Where is plastic surgery developing?
“She is switching to endoscopic methods of intervention - fewer incisions, less trauma. Let's take the forehead, for example, if earlier we made an incision from temple to temple, now three punctures are made and everything is tightened. And it heals faster. Now we are also switching to these methods and will do endoscopic forehead lift, temple lift, and chest lift. Here, look at this woman ( Nikolai Konstantinovich shows a photograph

), she’s about sixty, she had liposuction of her chin, a facelift, she took off the bandages and gasped - she didn’t have a chin all her life, and she always had a complex about it.
Now she has a chin, the wrinkles are gone. – How does a person live his whole life as he is, and then suddenly decide to go to a plastic surgeon?
– It’s very good when a woman prolongs her youth.
At least 10 years off is already good. I had a woman whom I operated on for the first time when she was 65 years old. We performed the second operation on her at 68 and a half years old and the third at 75 years old, it was a facelift. At the age of 69, the patient had her first lift, the result was positive. – Why at that age?!
- They want to live, they don’t want to grow old.
Women want to live beautifully, not sit on a bench in the yard. – Is their life changing?
- Certainly!
First of all, they become alive. They call and say, “I’m so many years old, will you perform an operation on me?”, I answer, if you’re “alive,” I’ll do it. The heart and blood pressure are normal, then we’ll do it. – Will you undergo plastic surgery for yourself?
- I did my eyes.
Of course I will again. I want to give myself a lift. I have a normal and natural attitude towards this. People come and look at me and see what I look like. – You’re like Pygmalion, who creates Galatea
– Yes, probably –
Do men also come to plastic surgeons?
- Certainly!
We also do eye lifts, but more noses. They leave rejuvenated and satisfied. Basically, these are well-known men in the city and region who occupy positions. Men come who are over 70, and we do things for them who are just over 50. They occupy a high position, they have an athletic physique, and there are already age-related changes on their faces, we do lifting. The person changes immediately. – That is,
when I’m 60, I can come to you and look younger - You don’t have to wait until you’re 60, you have to come when you’re 40-45 years old.
We'll make it thirty. – What is the canon of beauty for you?
- I don’t have a canon to make you look like someone.
When I go for an operation, I put my soul into it, I try to do it beautifully so that the patient is satisfied with the operation. A plastic surgeon is an artist. – Nikolai Konstantinovich, what would you wish to the visitors of the Siberian Medical Portal?
– Don’t be afraid to come for surgery. Find your doctor and trust him. Be young and beautiful!
