What is anti-Mullerian hormone?

Anti-Mullerian hormone
is a glycoprotein that limits excessive cellular growth in the reproductive organs of both women and men. This type of hormone is prescribed and monitored in the treatment of infertility.

The AMH hormone works even at the stage of embryonic development. It ensures the differentiation of the sex of the embryo into male or female. This protein substance provides control over spermatogenesis in men, as well as over folliculogenesis (ovum maturation) in women. The value of this substance is important when monitoring ovarian tumor processes; it is a kind of marker of this pathological condition.

An interesting fact is that at the stage of embryonic development this hormone “works” only in boys, being produced by the Sertoli cells of the testicles. But in girls, this hormonal peptide begins to be synthesized only after birth by the granular cells of the ovaries.

AMH has a number of significant differences from other biologically active substances.

The main ones include:

the protein base is a glycoprotein, while the other androgens, estrogens and progesterone are steroids;
exists in both the female and male body;
AMH production is not controlled by gonadotropic hormones.

Anti-Mullerian hormone (AMH) is a biologically active substance that is produced by ovarian granulomatous cells and Sertoli cells. The peak of AMH synthesis occurs during the reproductive period. With the end of menstruation, its level in the blood decreases to almost zero.

Indications and contraindications for analysis

When is a blood test for anti-Mullerian hormone necessary:

diagnostic process when using assisted reproductive technologies together with follicle-stimulating hormone (FSH) so that the ovarian reserve can be assessed and the body’s likely response to the applied hormonal stimulation of ovulation schemes can be predicted;
determination of tactics for managing women with infertility;
detection of germ cell tumors of the ovaries;
assessment of testicular function in men;
if you suspect early or delayed puberty;
during early menopause;
to monitor the effectiveness of cancer treatment and early diagnosis of relapses;
to establish sex in hermaphroditism;
when it is necessary to assess the possibility of pregnancy naturally;
to assess the degree of probability of conception during IVF;
if the couple has a history of unsuccessful IVF attempts.

The analysis is indicated for older patients in preparation for IVF. At low levels, the likelihood of fertilization decreases. If it is not confirmed that eggs are produced and ovulated in the body, IVF may be denied. In such a situation, patients are offered a donor program.

The analysis cannot be taken during the period of acute respiratory viral infection, during exacerbation of chronic pathology, or after excessive physical and psycho-emotional stress. AMH testing is not mandatory for all women planning a pregnancy. It is prescribed only if there are certain indications.

Indications for analysis of AMH hormone

In reproductive medicine, an AMH test is mainly prescribed to assess ovarian reserve and before choosing an ovulation stimulation program. In addition, the study is used in the following situations:

infertility;
unsuccessful IVF attempts;
insufficient response to ovulation stimulation;
high level of FSH;
monitoring the effectiveness of antiandrogen treatment;

How to prepare for an AMH test?

An AMH test is best done on day 3 of the cycle. Three days before the test, you need to provide your body with physical and psychological rest: avoid stress and overwork, sleep at least 8 hours a day. If you are taking any medications, you should inform your doctor. An AMH hormone test is not performed if you have ARVI or influenza.

High and low AMH

Anti-Mullerian hormone is normally found in a woman’s blood at a concentration of 1-2.5 ng/ml. These are average values. The norms depend significantly on age. Over the years, the concentration of AMH in the blood gradually decreases in women.

Elevated AMH may indicate the following conditions:

polycystic ovary syndrome;
delayed sexual development;
oncological diseases.

Possible causes of low AMH:

decline of reproductive function, menopause;
decreased ovarian reserve;
obesity.

Is it possible to increase AMH?

Often women who have low AMH are interested in how to increase the level of this hormone in the blood. They mistakenly believe that such actions will have a positive effect on reproductive function.

In fact, AMH is only an indicator, not a regulator of ovarian activity. It can be used to determine ovarian reserve, just as a thermometer can be used to measure body temperature.

If you artificially increase the level of AMH in the blood by taking medications, ovarian function will not change. Just as your body temperature will not change if you lower the thermometer into hot water.

IVF with low AMH

A woman should not worry about high or low AMH levels in her blood. All you need to know is that existing deviations from the norm do not mean that you will not be able to have children.
